Conflict and interaction checks

After individual analysis, interaction assessment examines whether a plausible mechanism is relevant to the actual treatment, exposure and person-specific context.

Methods, evidence and interpretation

01

Separate pharmacokinetic from pharmacodynamic mechanisms

Pharmacokinetic interactions alter absorption, distribution, metabolism or excretion, potentially changing exposure to a drug or active metabolite. Pharmacodynamic interactions alter effects at a given exposure, for example through convergent biological pathways or overlapping toxicities. A shared target is not enough to infer either clinical harm or synergy. Build distinct records for the mechanism, measured exposure change and observed clinical outcome; those claims require different evidence.

02

Distinguish inhibition, induction and time-dependent effects

Reversible inhibition depends on inhibitor concentration and mechanism. Time-dependent inhibition can involve enzyme inactivation and recovery through enzyme turnover. Induction involves changes in enzyme expression and activity and may evolve on a different timescale. Consequently, a short assay, a single sampling time or an apparent washout cannot answer every interaction question. Evaluate parent compounds and relevant metabolites, and document pretreatment and observation periods.

03

Connect in vitro potency to plausible human exposure

An in vitro inhibition signal needs an exposure bridge before clinical interpretation. Distinguish total plasma, unbound plasma, intracellular and intestinal concentrations; they are not interchangeable. Compare inhibition constants and concentrations in compatible molar units. Binding to assay proteins can affect measured potency. Screening models use defined assumptions to identify whether further investigation is warranted; crossing or not crossing a model threshold is not a universal safety verdict.

04

Investigate conflicting evidence rather than voting on it

When studies disagree, first compare chemical preparations, species, formulation, assay system, exposure, treatment duration and endpoints. A parent compound and a multi-ingredient extract are not equivalent exposures. A negative study may be uninformative if the tested concentration was too low or the relevant mechanism was not measured. Keep disagreements linked to their context, and distinguish failure to detect an effect from evidence that a meaningful effect is absent.

05

Make model-based extrapolation auditable

A physiologically based pharmacokinetic model can connect mechanistic processes with organ-level exposure, but the conclusion depends on the model’s intended use. Identify parameter sources, qualification datasets, sensitivity analyses and limits of extrapolation. Report model versions, assumptions, predicted changes and uncertainty. FDA’s reporting framework does not make every PBPK analysis acceptable: its regulatory use depends on the quality, relevance and reliability of the particular evidence package.

06

Preserve uncertainty at the clinical boundary

Interaction evidence for many supplements and cancer therapies remains limited. Absence from a database, a favorable simulation or a label such as “natural” does not establish compatibility. A research output should distinguish a documented interaction, a plausible mechanism and an unresolved question. Real-world assessment requires the complete treatment and supplement history and qualified clinical review; these educational pages do not determine whether a combination is appropriate.

A simplified inhibition sensitivity model

Explore reversible inhibition of one elimination pathway. The input r = Iᵤ/Kᵢ is an arbitrary unbound inhibitor-to-inhibition-constant ratio, not a drug dose.

AUC ratio = 1 / [(1−fₘ) + fₘ/(1+r)].135791100.250.50.751AUC ratioFraction eliminated through this pathway fₘ1.88× Illustrative AUC ratio

Assumptions: linear pharmacokinetics, constant unbound inhibitor exposure, one affected clearance pathway, unchanged bioavailability and other clearances. The curve varies fₘ while holding r fixed.

This pedagogical equation is not an ICH regulatory decision threshold, PBPK model or prediction for any named drug. It omits induction, time-dependent inhibition, active metabolites, gut effects and transporters. A ratio of 1 here does not establish safety.
